Using - A Caregiver's Guide

A Caregiver's Guide is distributed nationally in Canada to Hospice Palliative Care programs and services in English, French and Inuit (Nunavut) languages through the Canadian Hospice Palliative Care Association's, CHPCA Marketplace, which is accessible at www.chpca.net.

This session explores the origins of A Caregiver's Guide, including the book's use as a tool in helping co-manage care with families. It also examines effective uses for A Caregiver's Guide across settings of care (i.e., home, hospital, hospice, long-term care/continuing care).
